Job Overview We are seeking a compassionate and dedicated Medical Social Worker to join our Adult Day Health Care (ADHC) team. The ideal candidate will play a crucial role in providing support to patients and their families, helping them navigate the complexities of healthcare systems while addressing emotional, social, and financial challenges. This position requires strong interpersonal skills, a deep understanding of behavioral health, and the ability to work collaboratively with other healthcare professionals. Key Responsibilities: - Interviewing and screening all applicants/referrals to determine their appropriateness for the full assessment process and participation in the adult day health care (ADHC) program. - Perform psychosocial assessments and reassessments and develop individualized care plans with short- and long-term goals. - Document services provided and maintain up-to-date progress notes for each participant. - Collaborate with community resources to provide referrals as needed. - Serve as a liaison to participants' families, caregivers, and community agencies. - Participate in community outreach, education, and advocacy efforts. - Develop and implement discharge plans for participants, ensuring a smooth transition from the center. - Comply with health and safety regulations, including fire and disaster preparedness. - Supervise a team of 5-7 social workers - Attend orientation and follow all center policies and procedures. Skills and Knowledge: - Experience in clinical case management for chronic conditions. - Knowledge of community resources and treatment options. - Strong interpersonal, written, and verbal communication skills. - Ability to work within diverse cultural contexts with respect and sensitivity. - Excellent organizational skills and the ability to prioritize tasks. - Basic computer proficiency with PC-based software programs. Minimum Requirements: - Education: (a) Is licensed as a clinical social worker by the Board of Behavioral Sciences; or (b) Holds a Master of Social Work degree from a school of social work accredited by the Council on Social Work Education; or (c) Holds a master's degree in psychology, gerontology, or counseling from a accredited school, and has one year of experience providing social services in one or more of the fields of aging, health, or long-term care services. - ADHC, Adult Days Services or CBAS experience a plus - Supervisory: Minimum 2 years supervisory experience. - Language: Spanish speaking is preferred but not required. - Experience working with Seniors, dementia, disabilities and co-occurring disorders.
