Schedule: Full-Time
Bonus: $5,000 bonus over 1 year
Essential Duties:
Evaluate patients, develop effective treatment plans, and obtain approval for services from referring physician.
• Record daily treatment notes, weekly progress notes, recertification's, and discharge summaries.
Treat patients per plan of care.
• Communicate with supervisor and other health team members regarding patient progress and treatment plans.
• Participate in Care Conferences and Rehabilitation Meetings as needed.
• Instruct patient's families, nursing staff, and restorative aides as needed in maintenance/restorative programs, and caregiver training in preparation for discharge from therapy services.
• Recommend and facilitate the ordering of necessary durable medical equipment for patients to facilitate independence in mobility; i.e., braces, walkers, canes, elevated toilet seats and grab bars.

Successful Candidates:
- Actively Licensed by the State of Colorado
- Must be a graduate of an accredited Physical Therapy Program
- Experience in providing appropriate care in post-acute or long-term care environment (preferred)
- Ability to pass a criminal background check as well as Colorado CAPS background check.
Rate Range: $45.00-$50.00/hr DOE
