The Licensed Vocational Nurse (LVN) will provide essential nursing care and support to individuals served by Kings View who have health care conditions which require a Restricted Health Care Plan (RHCP) as outlined in Title 22. This role involves administering medications, monitoring health status, and assisting with personal support and daily activities to enhance the overall well-being of participants. The LVN will work closely with interdisciplinary team members to ensure a safe, supportive, and engaging environment for all clients.
How will I contribute
Respect client rights, independence, and personal choice at all times. Monitor and assess participants health status, reporting any changes to the supervising nurse or physician. Assist with personal care activities, including bathing, toileting, grooming, eating, and mobility support as needed. Develop and maintain positive relationships with participants, fostering a supportive atmosphere. Collaborate with the program team to implement individualized care plans and participate in case conferences. Maintain accurate medical records and documentation of care provided. Educate participants and their families about medication management, health conditions, and wellness strategies. Ensure compliance with health and safety regulations and maintain a clean, organized work environment. Report special incidents, neglect, exploitation, and suspected abuse according to applicable laws, including completing incident reports. Document daily progress notes in accordance with Title 17 and Title 22. Follow privacy regulations, maintaining confidentiality of client information. Develop positive relationships with outside agencies, care providers, and client families. Represent and promote the program to funding sources, community agencies, families, and related groups. Attend staff meetings and complete required continuing education as per Title 17 and Title 22. Develop, implement, and update Restricted Health Care Plans (RHCP) for individuals with health care conditions requiring special attention, including writing RHCPs for new intakes and providing staff training. Regularly review, and update RHCPs and re-train staff as necessary. Educate and train staff on various medical conditions and procedures, such as Seizure disorders, Asthma, Diabetes, G-Tubes, Catheters, Bloodborne Pathogens, etc. Work closely with administrative staff to create, implement, and maintain RHCPs. Provide excellent customer service when working with external stakeholders, including physicians, nurses, care providers, and families. Assist individuals in obtaining physician signatures for RHCPs and travel to doctors offices when necessary. Ensure timely and accurate billing related to RHCPs and request and secure authorizations for Regional Center funding. Perform nursing services, treatments, and preventive procedures as directed by physicians. Administer prescribed medications, G-Tube formula, fluids, and other medical treatments as specified in the RHCP. Administer PRN medications as prescribed, ensuring proper documentation and compliance with PRN regulations. Measure and record vital signs such as height, weight, temperature, blood pressure, pulse, and respiration as needed. Assemble and use medical equipment, including catheters and tracheotomy tubes. Ensure all electronic records are up to date with medication, doses, and physician notesMaintain a secure and organized system for storing medications.Perform other duties as assigned to support the overall function and success of the program.

High School Diploma (or Equivalent)Graduate of an accredited nursing programValid LVN license in the state of practice, Adult First Aid and CPR1-year previous experience in a healthcare or community setting.Class C Driving LicenseStrong understanding of medication administration and basic nursing care. Familiarity with caring for adults or seniors, as well as skills in providing personal care and support.Ability to assess individual health needs and respond appropriately to changes in condition. Ability to interact effectively with the program team and leadership and work with a diverse population of peers and clients, including those with behavior and medical support needs. Organization, time management, critical thinking, strong written and verbal communication, multi-tasking, active listening, reading comprehension, problem-solving, teaching, coordinating, innovating, and the strategizing skills and abilities necessary for service delivery.Positive attitude, strong work ethic, responsible, self-disciplined, creative, multi-culturally competent, empathetic, socially perceptive, flexible and adaptable. Must be at least age 18 and comply with program design personnel requirements: have a DOJ criminal background clearance, evidence of good health (verified by physical) and a negative TB test.Skills in crisis intervention and de-escalation are highly valued, as well as the ability to educate clients and families about health conditions and wellness strategies. Proficiency in electronic health records (EHR), data management and Restricted Health Care Planning.
