Community Hospice Care Consultant Job Description Job Title: Community Hospice Care ConsultantReports To: Vice President of Community Business DevelopmentDepartment: Hospice ServicesFLSA Status: Exempt Job Summary: The Community Hospice Care Consultant is responsible for driving hospice patient referrals through community engagement, education, and relationship-building with key referral sources. This role focuses on developing partnerships with physician offices, senior living communities, and other community organizations to increase awareness of hospice services and ensure patients receive the compassionate end-of-life care they need. The Community Hospice Care Consultant plays a critical role in executing business development strategies and expanding the organization's presence in the community. Key Responsibilities: Referral Management & Business Development: Develop and maintain strong relationships with physician offices, senior living communities, faith-based organizations, and other community referral sources.Identify and cultivate new referral sources to expand market reach and improve patient access to hospice care.Educate community organizations and healthcare providers on hospice services, eligibility criteria, and the benefits of end-of-life care.Track and analyze referral trends to optimize strategies and increase patient acquisition.Work closely with the Vice President of Community Business Development to implement growth strategies and achieve referral goals.Community Outreach & Education: Represent the organization at community events, health fairs, professional meetings, and networking opportunities to enhance brand recognition.Conduct in-service presentations for community organizations, senior care providers, and other potential referral sources.Collaborate with marketing teams to develop outreach programs and educational materials that promote hospice awareness.Stay informed about industry trends, regulatory changes, and best practices to support business development initiatives.Collaboration & Compliance: Work closely with the clinical and administrative teams to facilitate seamless transitions for hospice patients.Maintain accurate documentation and adhere to regulatory and compliance standards.Provide feedback to leadership regarding market opportunities, referral challenges, and competitive insights.Ensure all marketing and referral activities align with company policies and industry regulations.Qualifications: Education & Experience: Bachelor's degree in healthcare, business, marketing, or a related field preferred.Minimum of 2 years of experience in healthcare sales, business development, or marketing, preferably in hospice or palliative care.Strong knowledge of hospice regulations, Medicare/Medicaid guidelines, and the referral process.Skills & Competencies: Excellent communication, interpersonal, and relationship-building skills.Ability to work independently while collaborating effectively with interdisciplinary teams.Strong problem-solving skills and attention to detail.Proficiency in electronic health records (EHR), CRM systems, and Microsoft Office Suite.Goal-oriented with the ability to meet and exceed referral and revenue targets.Work Environment & Physical Requirements: Frequent travel to physician offices, senior living communities, community organizations, and other referral sources.Occasional evening or weekend work for community events or business development meetings.Ability to sit, stand, and move frequently in healthcare and community settings.Compensation & Benefits: Competitive salary with performance-based incentives.Comprehensive health, dental, and vision insurance.Paid time off and holiday benefits.Professional development and continuing education support.Equal Opportunity Employer: Mays Home Health, LifeSpring and Affiliates is an equal opportunity employer and does not discriminate based on race, gender, age, disability, or any other protected status under applicable law.
