This allows us to take a comprehensive, holistic approach to improving the lives of every person we serve - those who receive and those who reap the benefits of our innovative, responsive solutions.PRIMARY RESPONSIBILITIES: To make sure that the services of the Agency and the resources of the community are made available to the students and families at The Chimes School on both an individual and/or group basis and to assist the students being served in using these resources as they work collaboratively together to work towards the achievement of completing measurable and desirable social goals.
Collaborates and consults with other disciplines to ensure team understanding of student physical performance strengths and needs, through evaluation, educational program planning, and service delivery.
Evaluates the student's ability and formulates the student's academic and social profile through a variety of functional, behavioral, and standard assessments, skilled observation, checklists, histories, and interviews.
Synthesizes evaluation results into a comprehensive written report that reflects strengths and barriers to student participation in the educational environment; directs program development; and guides evidence-based intervention.
Develops appropriate intervention plans based on student needs and evaluation results.
Participates in IEP Individual Educational Plans meetings to review evaluation results, integrate findings with other disciplines, offer recommendations, and develop IEP goals and objectives and provides direct serv
Is responsible for completing and submitting program evaluations and discharge summaries for individuals to the proper agencies and administrators promptly.
Attends and participates in all annual IEPs, thirty-day, internal planning, and discharge meetings.
SERVICES TO SCHOOL PERSONNEL - Assessing students with mental health concerns. - Developing staff in-service training programs. - Assisting teachers with classroom management
SERVICES TO STUDENTS - Providing crisis intervention. - Develop intervention strategies to increase academic success. -Help conflict resolution and anger management - Helping children develop social and emotional skills.
EDUCATIONAL REQUIREMENTS:
Graduation from an accredited college or university with a Master's Degree in Social Work.
Must possess a valid and current LCSW or LMSW.
EXPERIENCE:2 years of clinical social work experience preferably in a school setting.
